• Completed and signed copies of tax returns, including W2s and schedules for the past three years
• Copies of most recent pay stubs (covering 30 days)
• Copies of the last two months’ bank statements for all deposit accounts
• Copy of driver’s license or identification card
• Copy of tribal enrollment
• $100 processing fee
